A few people have run into problems with getting the permissions for WindowsSignin.aspx in the AD provider setup and it's either been something on their server or with their browser settings. Because of that I created a small web application that either works or it doesn't but will at least let you know if the problem is in the provider and DNN or your server/browser.
Create a new virtual directory or website in IIS and extract these files into it. The first run through you should get a message like:
Welcome: \IUSR. Then edit the IIS permissions for default.aspx and uncheck Anonymous leaving only Integrated Windows Authentication. Then when you visit the page you should get Welcome: DOMAIN\Username.
